Best Buddies Forms

If you want to get a head start on our first club meeting on September 11th, please download the forms below. Fill each out and bring them along with you to the meeting. The first two forms are a Membership Application and College Buddy Matching Survey. These will be used to match you with a 'buddy'! The last is an optional application to become an officer. Only fill out if you are intersted in becoming an officer.
